I don't see anything wrong in your AD, nothing seem to be wrong with your citrix farm as others are ok. But make sure delete all profiles (local/roaming) and find SID in registry and delete that too.To delete profile on the citrix server do it this way: Control Panel > System and Security > System > System Properties > Advanced > User Profiles > Settings > highlight profile and delete.By resetting account to default in ADUC - didn't make any difference this definitely a user account issue.
